- Medication support according to valid prescriptions and the care plan
- Skin, pressure-area, mouth and personal care
- Positioning, mobility and comfort measures
- Nutrition and hydration support based on goals of care
- Emotional support and compassionate presence
- Family caregiver guidance and respite support
- Coordination with doctors and other palliative-care professionals when required

When to consider support
Hospice or palliative homecare may help when...
Daily care is becoming difficult for the person or family to manage alone.
A serious illness is causing increasing comfort, mobility or personal-care needs.
Family caregivers need guidance, reassurance or planned respite.
The person's preference is to spend more time at home with appropriate support.
Important
Hospice homecare is not an emergency service. Sudden severe symptoms or an emergency require urgent medical assessment. Medication prescribing and changes remain the responsibility of an authorised prescriber.
